Membership Billing Details
New Patients
- New patient memberships will be $120 per month for the first year and $100 per month thereafter
- Initial new patient visits are $350 and include the first month's membership fee
- Families with 4 or more PHW members within a household will receive a discount of $10 per month on each membership
Family Discount: Large families save with our household discount program.

Membership cancellation requires 60-day notice for continuity of care and can be processed through the patient portal or by calling one of our clinic locations.
